GNBA Clubhouse Event Application

By checking this box, I agree that I have read, understand and will follow the Clubhouse Guidelines and Clubhouse Checklist found as downloadable forms on the GNBA Website.
If I am scheduling a PRIVATE EVENT, by checking this box, I understand that before my event can occur, I will need to provide proof of $1,000,000 home owners liability insurance which can be scanned or photographed and emailed to: gnbaclubhouse@gmail.com.

If I am scheduling a PRIVATE EVENT, by checking this box, I understand that before my event can occur, I will need to mail two checks to GNBA Treasurer at P.O. Box 226, Niantic, CT 06357. The first check should be for $175 (donation for use of Clubhouse) The second check is a security deposit of $200 (to be shredded if Clubhouse is in proper condition post event per the Event Checklist). Please put the DATE OF YOUR EVENT in the comment section of the checks.
